Default SIP Proxy Server and PSTN

Ok the geeks on here draw your pocket protectors !

So I have been researching since couple of months ago about how to setup my own SIP proxy server plus how to connect it to the IP telephony PSTN. I already set up my Asterisk running fine in my Linux box.

I was reading that probably I need to make some FXS connections by getting a Sipura SPA-2000 gateway in order to connect my VoIP to the "real-phones" out there.

I would really appreciate some explanations about this.
    - It is possible to setup my own SIP proxy server?
    - It is possible to make calls to "real-phones" without passing through any commercial gateways or any SIP servers? or do I have to pass through some external gateways ?
